MEDICAL & LIABILITY

I, being aware of my own health and physical condition, and having knowledge that my participation in any exercise program may be injurious to my health, am voluntarily participating in physical activity with Leanne Moore (LIFT By Leanne, GLOW by Leanne).

Having such knowledge, I hereby release Leanne Moore (LIFT By Leanne, GLOW by Leanne), her representatives, agents, and successors from liability for accidental injury or illness which I may incur as a result of participating in the said physical activity. I hereby assume all risks connected therewith and consent to participate in said program.

Furthermore I agree that, if needed, I have received medical approval to begin this exercise program and am medically fit to do so.

I understand that I am the sole owner/user of my subscription and take responsibility for others who use the service through my subscription.
